The following Behavioral Health Care Providers receive some funding from the Four County ADAMhs Board to provide services to persons needing help on a sliding fee basis. Depending on family size and income, the individual may be responsible for paying a portion of the cost of their services with the ADAMhs Board reimbursing the agency for the remaining cost. The ADAMhs Board is entirely funded by two small property taxes, state allocations from the Ohio Department of Mental Health and Addiction Services, and federal grants. In the following list, the first address given is each provider’s main office. However, many of the providers have multiple offices throughout the four county area. The addresses and phone numbers are provided for all of the providers' service locations.
A Renewed Mind Behavioral Health

At their Defiance, Bryan and Wauseon offices, A Renewed Mind provides both intensive and non-intensive outpatient substance use disorder counseling services, as well as individual, group and family counseling, case management, medication-assisted treatment, and nursing/medical services for adult clients with substance use disorders.  The offices also provide mental health services, including: diagnostic assessment, outpatient individual, group and family counseling, and case management for youth and adults.​  Psychiatry services are also available for adults and youth who are ages 6 and up.

At their Napoleon facilities, the agency provides the same outpatient services that it offers in its other locations at Crisis Care and Counseling Center, 600 Freedom Drive.  A Renewed Mind also operates a residential treatment facility for men with addiction problems and provides ambulatory detox and medication assisted recovery at The Renewal Center, 1895 Oakwood Avenue.

​The agency also responds to requests for emergency crisis assessments for both mental health and addiction 24 hours a day, seven days a week.  The toll-free crisis hotline number is 800-468-4357.
Home-based mental health services are available in Defiance and Williams counties and school-based services are currently offered at the Independence Education Center, Bryan, Montpelier, Evergreen and Liberty Center schools with the goal of expanding into more districts.

Our integrated services include primary health care for adults and children, dental care, pharmacy, chiropractic services and behavioral health and substance abuse.  We accept all insurances (Medicare, Medicaid and commercial) and also have a sliding fee scale for all who qualify.  No patient will be turned away for inability to pay.  The Bryan Community Health Center is a part of Health Partners of Western Ohio, a federally qualified health center.

CCFA provides comprehensive services to those affected by abusive behaviors through treatment, prevention, education and advocacy.  General counseling services include anger management, bereavement, family, individual, intensive family-based, marriage, group, and play therapy.  CCFA serves both victims and offenders of domestic, physical and sexual assault.  The House of Ruth, a domestic violence shelter, operates a 24-hour domestic violence hotline.  The Four County SART Clinic provides forensic medical exams and may be contacted by calling the 24-hour hotline.  CCFA also provides evidence-based prevention and education programs.

Maumee Valley Guidance Center has offices in all four counties and is certified by the Joint Commission to provide a variety of outpatient mental health services for adults, children, adolescents and families.  Behavioral healthcare services offered include...

Provides safe, clean, decent and affordable housing services to persons residing in the four county service area (Defiance, Fulton, Henry and Williams) with qualifying mental health diagnosis.  The agency operates the following housing.
​Horizon Apartments in Bryan (10 one-bedroom apartments) offers 24 hour on-site monitoring.
Upton Apartments in Defiance (10 one-bedroom apartments) offers 24 hour on-site monitoring to qualifying mental health consumers who have a history of alcohol or drug addiction.
​Pontiac Apartments in Defiance is a 6-unit HUD 811 project that provides PRAC subsidy for six tenants.
​Another 30 units of single family/duplexes are also owned by the agency.
​A rent subsidy program is administered by the agency when funding is available.
​Emergency funds are sometimes available and may be accessed in the form of a loan if funding is available.

The P.A.T.H. Center

Operated by Northwestern Ohio Community Action Commission, the P.A.T.H. Center provides a soup kitchen and walk-in center.  Also on site is a homeless shelter and single room apartments.  Participants also have access to case managers who help them link up with services.

Quadco Rehabilitation Center

Provides work skills and job training/support for persons with disabilities.  Quadco offers a variety of work experiences that are operated by the center, job placements and job coaching, and other supports in the community.

Provides outpatient alcohol/drug counseling for adults and adolescents.  Residential treatment for women (Serenity Haven at 419/237-3103 in Fayette).  Certified and funded to provide mental health services, particularly for persons with both substance abuse and mental health problems, and can provide free psychiatric medications for qualifying patients.  Also offers prevention programming.
